Advance directive

The FMH and SAMS provide an advance directive in two variants as well as a credit-card sized notice for a wallet or purse.

Personal details as well as information about the trusted third parties can be electronically entered, stored and printed out in the Patient Decree (living will). However, we have opted against designing the document such that the entire content can be entered electronically. The aim is to underscore the importance of the document as a highly personal decree. With the exception of the signature, the notice card can also be electronically completed.
